On or about July 26, 2018:  At the worksite located at 1545 Community Way NE in Gainesville Georgia, an employee was exposed to caught between and struck by hazards while utilizing a rough terrain forklift as an elevated platform to reach the working area. The employee operating the forklift was working on the other side of the structure, and left the forks in the air.
