29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set. On or about July 26, 2018: At the worksite located at 1545 Community Way NE in Gainesville Georgia, an employee was exposed to caught between and struck by hazards while utilizing a rough terrain forklift as an elevated platform to reach the working area. The employee operating the forklift was working on the other side of the structure, and left the forks in the air.

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: On or about July 24, 2018: At the worksite located at 1545 Community Way NE in Gainesville, Georgia, employees were exposed to struck by and impalement hazards when the grinders that they were utilizing did not have the guards attached to them as recommended by the manufacture.
