29 CFR 1926.300(b)(3): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 20, 2018 at the worksite located at Stone Point Way Lot #143 in Buford, Georgia: a) An employee was exposed to possible amputation hazards when the employer failed to ensure that all guards on the cement mixing machine were on or in the closed position.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about July 20, 2018 at the worksite located at Stone Point Way Lot #143 in Buford, Georgia: a) Employees working from a scaffolding system was exposed to fall hazards of approximately 25 feet, when the employer failed to ensure that they were properly protected by fall protection measures as laid out in paragraphs (g)(1)(i)-(vii) of this section.
