29 CFR 1926.453(b)(2)(v):   A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.  a)  91-93 Horse Hill Road, Cedar Knolls, NJ.  Employee working from the JLG 400S Telescopic Boom Lift was exposed to being catapulted from the lift as no lanyard was attached to the boom or basket.                                        Violation observed on or about 07/21/2018.

29 CFR 1926.1053(b)(13):   The top or top step of a stepladder was used as a step:   a)  91-93 Horse Hill Road, Cedar Knolls, NJ.  Employee placing a lightweight steel dormer on the deck of this commercial building was exposed to a fall from the 6 foot stepladder as he was working from the top step of the ladder.                                      Violation observed on or about 07/21/2018.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  91-93 Horse Hill Road, Cedar Knolls, NJ.  Employees placing a lightweight steel dormer on the deck of this commercial building were exposed to over 17 foot falls to the ground below as no methods of fall protection were being used.                                      Violation observed on or about 07/21/2018.   Freehold Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1254289, citation number 1, item number 2, and was affirmed as a final order on 02/12/2018, with respect to a workplace located at 1340 Hamilton Street, Somerset, NJ.
