29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section; a) On or about 07/17/18 in the Large Votator room; employee was exposed to an amputation hazard during the clearing of clogged transfer line from K-15 when employer did require the use of the written LOTO procedure- Lg. Vot. 2b before commencing the work. b) On or about 07/26/18 throughout the production area; employees were exposed to an amputation hazard while engaged in the service and maintenance of production equipment, such as but not limited to cleaning, clearing jams, set up, change over, when the employer did require the use of written LOTO procedures such as but not limited to Lg. Vot. 1a, Lg. Vot 2a, Lg. Vot. 3 before commencing the work. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation; a) On or about 07/17/18 in the Large Votator room; employee was exposed to an amputation hazard during the clearing of clogged lines when employer did not train to the Authorized employee level so that the magnitude of the energy, the methods for energy isolation and the means to necessary to control the hazards were provided. b) On or about 07/23/18 in the Large TA's room; employee was exposed to a laceration hazard during the inspection and cleaning of the filler head when the employer did not train so that methods for energy isolation were implemented. c) On or about 07/26/18 throughout the production area; employees were exposed to an amputation hazard while engaged in the service and maintenance of production equipment, such as but not limited to cleaning, clearing jams, set up, change over, when the employer did not train these employees as Authorized employees so that they recognized the magnitude, methods and means for control of hazards. ABATEMENT DOCUMENTATION REQUIRED
