29 CFR 1910.217(b)(7)(v)(d): The control systems shall be designed to require release of all operators' hand controls before an interrupted stroke can be resumed. This requirement pertains only to those single-stroke, two-hand controls manufactured and installed on or after August 31, 1971. a. On or about July 18, 2018 at the facility located at 2685 Universal St Oshkosh, WI 54903, the employer did not properly guard the point of operation of the L&J Press Corporation Mechanical Power Press serial number 33217. The control system did not require the release of both of the operators' hand controls before an uninterrupted stroke could be resumed.

On or about July 18, 2018 at the facility located at 2685 Universal St Oshkosh, WI 54903, the employer did not properly guard the point of operation of the L&J Press Corporation Mechanical Power Press serial number 33217. The fixed barrier guard was not securely attached to the frame of the press.

29 CFR 1910.217(f)(2): Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed. a. On or about July 18, 2018 at the facility located at 2685 Universal St Oshkosh, WI 54903, the employer did not provide adequate instruction on the correct operating procedures for an employee operating a mechanical power press. The employee suffered a partial amputation of the left middle finger after three days of operating the mechanical power press.
