29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: (a) Facility, 4801 Grand Avenue, Suite A, Warehouse, Pittsburgh, PA: On or about July 25, 2018 there was not adequate clearances in the aisles for the powered industrial trucks to travel. An electrically powered industrial truck could not be charged because access to the charging station was blocked.

29 CFR 1910.136(a): The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures: (a) Facility, Warehouse, 4801 Grand Avenue, Suite A, Pittsburgh, PA: On or about July 25, 2018 employees working in the warehouse were not wearing the required protective footwear as determined by the employer's hazard assessment.
