29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork. A) Steve Lennon on or about July 25, 2018 conducting work at 550 E South Boundary Street, Perrysburg, OH: The employer did not ensure a wood pallet, was securely attached to the forks. The unsecured platform was at a height of 20ft while an employee installed nuts and bolts underneath an athletic bleacher.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. A) Steve Lennon DBA Lennon Construction on or about July 25, 2018 at the address of 550 E Boundary Street, Perrysburg, OH: The employer did not ensure an employee was protected from a fall while working from a makeshift mobile scaffold platform (wood pallet). The employee was exposed to a 20ft fall to the level below while installing bolts and nuts under the athletic bleachers.
