The pump jack scaffolding systems was not inspected for visible defects by a competent person before each work shift as demonstrated by much of the scaffolding fall protection netting being secured by single strand wire and the use of an extension ladder as the work bench and securing point for fall protection netting. Violation observed on or about 07/21/2018.

29 CFR 1926.451(g)(4)(ix): Midrails, screens, mesh, intermediate vertical members, soild panels, and equivalent structural members of a guardrail system were not capable of withstanding, without failure, a force to applied in any downward or horizontal direction at any point along the midrail or other member of at least 75 pounds for guardrail systems with a minimum 100 pound toprail capacity, and at least 150 pounds for guardrail systems with a minimum 200 pound toprail capacity: a) 91-93 Horse Hill Road, Cedar Knolls, NJ. Employees were exposed to falls of up to 15 feet to the ground below as much of the pump jack scaffolding's guardrail netting was secured using only single strand wire and to an extension ladder used as a work bench which is a system not designed to prevent falls given the forces generated when one may fall into the netting. Violation observed on or about 07/21/2018.

Employee was exposed to falls of approximately 18 1/2 feet to the ground below as the worker was standing on the work bench above the scaffold platform without using any fall protection and employees on the work platform of the scaffold were exposed to 15 foot falls as the pump jack scaffolding's work platform was missing end rails. Violation observed on or about 07/21/2018. Classic Stucco and Stone, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1161441, citation number 1, item number 2, and was affirmed as a final order on 09/21/2016, with respect to a workplace located at 25 Mclean Blvd, Paterson, NJ.
