29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: During demolition work at 600 S. Main St. (elementary school), Monroe City, a crane operator, who was involved in an accident at the site during July, 2018, was not trained on crane operation. Crane operators were not certified. (Note: crane operation training and certification may be reviewed at 1926.1427. Although the crane training and certification requirement of the standard is being re-written, training to operate a crane is required under 1926.21(b)(2).) (This violation was corrected by the employer discontinuing crane operation.)

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift (Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated): At the 600 S.
