29 CFR 1910.140(c)(18): Personal fall protection systems was not inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components were not removed from service:  a. The securing pin to one of the vertical structural members of the fall protection anchorage system had become displaced.  The system had not been removed from service until the defective condition was repaired.  (The opposite side pin was not secured by a cotter pin, but was not displaced.)  b. The pins designed to lock the anchor system in a use position were unable to be used because the slots had become warped.  Undersized pins that could be easily displaced were used instead.  The system had not been removed from service until the defective condition was repaired.  c. Welds on the vertical structural members were damaged.  The system had not been removed from service until the defective condition was repaired.

29 CFR 1910.178(o)(2): Load(s) were being handled which exceeded the rated capacity of the industrial truck(s):  During the rolling of trailers, forklifts used to lower ("catch") the trailers after they reach the peak or break point of the roll, have had the rear wheels raise from the floor, indicating that the weight capacity was exceeded.  (Note: the specification plate with capacities was not legible on the 80 and 100.)
