29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  Instance a. Site - On or about July 24, 2018, employees engaged in framing activities were not utilizing fall protection and were exposed to falls of over 23 feet.  Instance b. Site - On or about July 24, 2018, the employer failed to provide guard rails around the open sided staircase to prevent employee falls of over 10 feet.  Instance c. Site - On or about June 24, 2018, employees were exposed to fall hazards where the window openings on the second floor level were not guarded or protected to prevent employee falls of over 16 feet.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided:  Site - On or about July 24, 2018, the employer failed to provide a ladder or stairs at the employee point of access into the building where the break of elevation exceeded 27 inches.
