29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a. On or about July 20, 2018 around 11:34 am, at 375 Cypress Drive, Bay Lake, FL, an employee suffered a work related injury and was hospitalized. A superintendent was notified of the in-patient hospitalization on 7/21/18 and the employer did not report the in-patient hospitalization to OSHA until July 23, 2018 at 2:30 pm.
