29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about July 25, 2018, on the north side of the building, an employee was not protected from falling while climbing a scaffold to access an unprotected edge of the building to install windows, exposing the employee to fall hazards exceeding 10 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about July 25, 2018, on north side of building, an employee was exposed to fall hazards exceeding 6 feet while installing windows on the edge of building without fall protection.
