On or about 07/26/18, on the floor of 185-11 73rd Ave. Fresh Meadows, NY    Employee was cutting metal with a Makita 241 4B cut-off machine without a guard.    ABATEMENT VERIFICATION IS REQUIRED.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   A.           On or about 07/26/18, at site of 185-11 73rd Ave. Fresh Meadows, NY    Employees working on the fabricated frame scaffolds were exposed to the hazard of falling approximately 12 ft. to the ground. The working platforms were installed with only two or three scaffold planks.       ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

On or about 07/26/18, scaffolds at 185-11 73rd Ave. Fresh Meadows, NY    Employees working on fabricated scaffolds approximately 12 ft. while bricklaying 2-story single family house were exposed to the hazard of falls. Guardrails or personal fall arrest systems were not provided.  ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
