29 CFR 1910.67(c)(2)(v):  A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift.  a)Residential home at Prospect Ave, in Cliffwood Beach, NJ, 07735: Employer did not ensure employees working from an Altec LR11155 aerial lift truck, serial number 0293BH0162 operating at approximately 40 feet were wearing a body harness connected by a lanyard to the device, while performing tree removal, on or about 7/10/18.  NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
