29 CFR 1910.147(d)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: In the shipping/Dock area, the employer did not develop written lockout/tagout procedures describing how to lockout and control hazardous energy sources for affected and authorized employees for machines, such as but not limited to, the Genie controller to the Southern Overhead Door, serial number 16277511145, model number OPGSXH5003L, which exposed employees to the hazard of being caught in or struck by the equipment. W.R. Meadows, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d), which was contained in OSHA inspection number 1250879, Citation Number 1, Item number 1, and was affirmed as a final order on February 12, 2018, with respect to a workplace located at 300 Industrial Drive, Hampshire, Illinois 60140.
