29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a). At the worksite - On or about July 24, 2018, employees operated a reciprocating saw and a pneumatic nail gun without the use of eye protection, thus exposing the employees to a struck-by hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a). At the worksite - On or about and times prior to July 24, 2018, employees worked from the building's top plates and floor joists without the use of a fall protection system, thus exposing employees to a fall hazard up to approximately 12.2 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  a). At the worksite - On or about and times prior to July 24, 2018, the employer did not provide fall protection training to employees who performed wood framing activities from the building's top plates and floor joists, thus exposing employees to a fall hazard up to approximately 12.2 feet.
