29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use.  a). Westside of the building - On or about July 25, 2018, employees were elevated on a personnel platform (i.e. wood pallet) that was supported by a Kubota Compact Truck Loader (model: SVL90-2) that was not designed to support personnel platforms, thus exposing employees to a fall hazard of approximately 11 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access.  a). Westside of the building - On or about July 25, 2018, employees climbed the end frames of a fabricated frame scaffold to access/egress the working level of the scaffold, thus exposing employees to a fall hazard of approximately 17.2 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.  a). Westside of the building - On or about July 25, 2018, employees worked on a fabricated frame scaffold without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 17.2 feet.

Westside of the building - On or about July 25, 2018, the employer did not have employees who worked from a fabricated frame scaffold trained by a qualified person in how to recognize hazards associated with fabricated frame scaffolds and the procedures to control or minimize those hazards, thus exposing employees to a fall hazard of approximately 17.2 feet.
