29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a. On or about July 27, 2018, in the warehouse area, employees were required to walk out onto a raised pallet or rack system over eight feet high and were not provided the appropriate protection to prevent a fall. In the absence of a guardrail and/or safety net system, the personal fall arrest system provided by the employer for employee use would not prevent employees from making contact with the floor surface in the event of a fall while accessing the rack system.

On or about July 27, 2018, in the warehouse area, employees were required to walk out onto a raised pallet or rack system over eight feet high and were not provided the appropriate fall arrest system. The personal fall arrest system provided by the employer for employee use would not prevent employees from making contact with the floor surface in the event of a fall while accessing the rack system.
