29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On 7/23/2018, An employee of Edgar Vega were working on the ground level of a residential building located at 707 Canterbury Drive, Warrenton, MO.   Protective Helmets were not worn by the employee exposed to hazards of falling objects from the roof above.

29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On 7/23/2018, employees of Edgar Vega were working on the roof of a residential building located at 707 Canterbury Drive, Warrenton, MO.  Eye protection was not used by employees involved with using pneumatic nail guns during roofing operations.

29 CFR 1926.501(b)(13): "Residential construction." Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 7/23/2018, employees of Edgar Vega were working on a residential building located at 707 Canterbury Drive, Warrenton, MO.  Employees were installing shingles approximately 10 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.

29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails were not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 7/23/2018, employees of Edgar Vega were installing shingles on the roof of a residential building located at 707 Canterbury Drive, Warrenton, MO.    The ladder the workers were using to access the roof was not extended three feet beyond the upper landing surface of the roof.

1926.1053(b)(22): An employee did carry any object or load that could cause the employee to lose balance and fall.  On 7/23/2018, employees of Edgar Vega were installing shingles on the roof of a residential building located at 707 Canterbury Drive, Warrenton, MO.   Employees were observed carrying supplies, leaf blower, and circular saw up the ladder, which could have caused them to loose balance and fall.
