On 7/23/2018, employees of Edgar Vega were working on a residential building located at 707 Canterbury Drive, Warrenton, MO. Employees were installing shingles approximately 10 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.

The ladder the workers were using to access the roof was not extended three feet beyond the upper landing surface of the roof.
