Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     a) On or about July 27, 2018, at the above reference inspection site, employees performing roofing work while on the 4/12 pitched residential roof were exposed to falls of approximately 8 feet due to the lack of fall protection.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  a) On or about July 27, 2018, at the above reference inspection site, employees were utilizing an extension ladder to gain access/egress to a residential roof. The employer failed to ensure the ladder extended at least 36 inches above the surface being mounted. Employees were thereby exposed to falls of up to 8 feet.   There is no abatement certification of documentation required for this item.
