29 CFR 1915.73(e): When employees were working near the unguarded edges of decks of vessels afloat, they were not protected by personal flotation devices meeting the requirements of 29 CFR 1915.158(a).  a)    Pier 3, Norfolk Naval Station: On or about 07/27/2018, employees engaged in diving operations from the stern of their vessel were not wearing a personal flotation device.    No abatement certification or documentation required.

29 CFR 1915.74(b): Gangways meeting the requirements of 29 CFR 1915.24(a)(1), (2), (9), (10), and (11) were not provided for access from wing wall to vessel, or were not provided when two or more vessels, other than barges or river towboats, were lying abreast.  a)    Pier 3, Norfolk Naval Station: On or about 07/27/2018, employees were required to use a gangway 1 foot wide x 16 feet long with a guardrail on one side to access and egress the vessel.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.30(a): The employer did not maintain a separate OSHA 300 Log for each establishment that was expected to be in operation for one year or longer.  a)    Seaward Marine Services, LLC: On or about 07/30/2018, the employer did not maintain a separate OSHA 300 log for each establishment. Employer provided OSHA 300 logs for 2015, 2016 and 2017 that contained injuries and illnesses from their other establishments.   No abatement certification or documentation required.
