29 CFR 1926.451(g)(4)(iii): Midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members were not installed between the top edge of the guardrail system and the scaffold platform. On 7/25/2018, Reed Exterior Construction LLC was installing siding on a residential building located at 934 Mule Creek Dr. in Wentzville, MO. The owner and an employee were working from a pump jack scaffold on the rear of the residence. The scaffold did not have midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members installed between the top edge of the guardrail system and the scaffold platform.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. On 7/25/2018, Reed Exterior Construction LLC was installing siding on a residential building located at 934 Mule Creek Dr. in Wentzville, MO. The owner and an employee were working from a pump jack scaffold on the rear of the residence.
