29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Warehouse/Packaging Area: Procedures were not developed, documented and utilized for employees that provided maintenance to machines, such as, but not limited to Sanko FC1000, on or about 7/16/18.    b) Warehouse/Cartoner Area: Procedures were not developed, documented and utilized for employees that cleared jams on machines such as, but not limited to Yeaman Horizontal Cartoner, on or about 7/19/18.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources:    a) Warehouse/Packaging Area: The employer did not provide authorized employees lockout or tagout devices to use when performing maintenance on machines, such as, but not limited to Sanko FC1000, on or about 7/16/18.    b) Warehouse/Cartoner Area: The employer did not provide authorized employees lockout or tagout devices to use when clearing jams on machines such as, but not limited to Yeaman Horizontal Cartoner, on or about 7/19/18.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees:    a) Warehouse/Packaging Area: The employer did not provide lockout tagout training to employees that provided maintenance to machines, such as, but not limited to Sanko FC1000, on or about 7/16/18.    b) Warehouse/Cartoner Area: The employer did not provide lockout tagout training to employees that cleared jams and maintenance on machines such as, but not limited to Yeaman Horizontal Cartoner, on or about 7/19/18.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
