29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On 7/26/2018, employees form Girod Builders, LLC were installing guttering on a building located at 1047 E. Highway N in Wentzville, MO. The employees were using a Genie Aerial Lift, Model S-65, to access the roof. The employees were not wearing body belts with a lanyard or personal fall arrest systems that were attached to the boom or basket while utilizing the aerial lift.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On 7/26/2018, employees form Girod Builders, LLC were installing guttering on a building located at 1047 E. Highway N in Wentzville, MO. Three employees were connecting guttering together near the edge of the roof, which was approximately thirty feet above the ground, without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
