On or about July 25, 2018, the employee using a nail gun to install roofing material was not wearing eye protection, being exposed to eye injuries from flying nails.

On or about July 25, 2018, employees installing a re-roof on a 4:12 pitch roof were not protected from falling up to a 22.9 feet to the ground by the use of a fall protection system.
