On or about July 24, 2018, throughout facility, employees were observed operating a Caterpillar forklift Model No. D50, a Caterpillar forklift Model No: D60 and a Crown Electric forklift that did not have a seatbelt attachment, exposing employees to crushing hazards.

On or about July 24, 2018, the employer did not have an established lockout program consisting of an energy control procedures, employee training and periodic inspections for machine or equipment such as but not limited to a Powermatic dust collector, Vertical door craft, milling machine and shear and other similar equipment, thereby exposing employees to unexpected energizing, startup or release of stored energy hazards.

On or about July 24, 2018, the employer did not develop and implement a written hazard communication program which consisted of a written program including an explanation of their workplace labeling system used by the employer, the safety data sheets, pictograms and how employees could obtain and use the appropriate hazard information. Employees used hazardous chemicals such as but not limited to propane, wood glue, silicone, liquid nail and oils while working in the facility.
