The employees were not utilizing a fall protection system or fall arrest harnesses.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 90 Courier Dr: On or about July 27, 2018, employees were exposed to a fall of up to 10 feet while using a 8 foot Werner extension ladder to gain access from the garage roof to the second story roof to replace shingles. The ladder did not extend above the edge of the roof.
