29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about July 25, 2018, the employer failed to develop and implement a written control of hazardous energy (lock out tag out) program. Employees are performing various tasks, such as but not limited to the change out needles and sharpening machine blades on the various quilter, border and surger machines and are exposed to hazardous electrical and pneumatic energy. NOTE: The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1092718, citation 1, item 1, which was affirmed as final order on December 16, 2015, with respect to the workplace located at 161 Grand Valley Ave., Orwell OH 44076.
