29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     1. An employee performing roofing work on a sloped roof is not protected from fall hazards through the use of a fall protection system.      The employer fails to assure effective protection against falls as required by the standard, as demonstrated on or about July 23, 2018, at 620 El Camino Real, Naples, Florida, 34119, on the East side of the residential property, when an employee installing tiles on a roof with a pitch of 5:12, without the use of conventional fall protection, was exposed to a fall hazard greater than 11-feet to ground level.    a) Ad-Ler Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1295589, citation number 01, item number 001, with respect to a workplace located at 11593 Bitola Drive, Odessa, Florida, 33556, and was affirmed as a final order on 07/16/2018 as a 2nd Repeat item.    b) Ad-Ler Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1293156, citation number 01, item number 001, with respect to a workplace located at 3368 Forsythia Drive, Florida, 33556, and was affirmed as a final order on 07/16/2018 as a 2nd Repeat item.      c) Ad-Ler Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1291889, citation number 01, item number 001, with respect to a workplace located at 3399 Forsythia Drive, Odessa, Florida, 33556, and was affirmed as a final order on 04/09/2018 as a 2nd Repeat item.    d) Ad-Ler Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1139096, citation number 01, item number 001, with respect to a workplace located at 8219 Encleve Way, Sarasota, Florida, 34243, and was affirmed as a final order on 05/12/2016 as a 2nd Repeat item.
