29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  (a) 1915 Shady Oak Road, Mount Joy, Pennsylvania - The employer did not certify that the citations had been abated within 10 days of the abatement date, on or about September 27, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:    a)  1915 Shady Oak Road, Mt. Joy PA - Employees were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas, on or about July 12, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section (fall protection training) by preparing a written certification record which contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) 1915 Shady Oak Road, Mt. Joy PA - The employer did not prepare a written certification verifying that employees had been trained in hazards related to falls from elevations, on or about July 12, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
