29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access:    On or about 7/30/18     Location: 3926 White Plains Rd Bronx NY 10466    a) Employees were exposed to fall from height hazards due to no proper access for a scaffold. Employees used the crossbraces as means of access.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about July 30th, 2018    Location: 3926 White Plains Rd Bronx NY 10466    a) Employees were exposed to fall from a height of approximately 9ft while repairing a roof. CSHO observed employees using the 32' Louisville aluminum extension ladder which was extend a maximum of 1 ft. above the upper landing surface to which the ladder is used to gain access.
