29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury: a) Conrader Holdings, LLC at 749 E. 18th Street in Erie, PA. On or about July 31, 2018, an energy control program (lockout/tagout program) consisting of documented procedures, training, inspections/audits and lockout hardware had not been developed. Employees performed servicing and maintenance on machines .

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) Conrader Holdings, LLC at 749 E. 18th Street in Erie, PA. On or about July 31, 2018, and at times prior there to, an amputation was not reported to OSHA within 24 hours. The accident, and resulting amputation, took place on or about July 12, 2018. The incident was not reported to OSHA until July 27, 2018.
