On or about July 27, 2018, the employer allowed a subcontractor employee to climb into a filled garbage dumpster that was attached to the forks of the telescoping all terrain vehicle the employer was operating (JCB 510-56, Serial #5CG7GHJ2734912) so that the subcontractor employee could be lifted to the roof of a building that they were re-roofing (Christ Church Episcopal, 21 Aurora St., Hudson OH).  The subcontractor employee was standing in the filled dumpster at a height of approximately 30+ feet and was working out of the dumpster.  The subcontractor employee was exposed to a fall from height hazard as well as misc. other injuries by standing in a dumpster full of scrap metal and debris.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about July 27, 2018, the employer failed to ensure that subcontractor employees who were working on a roof at a job site (Christ Church Episcopal, 21 Aurora St., Hudson OH) were protected from a fall from height hazard of approximately 30-40 feet.  The roof had a pitch of 9/12 and employees were reroofing the entire facility so the heights varied.  Subcontractor employees were not wearing a personal fall protection harness nor was their an alternative fall protection system in place.      NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1108149, citation 1, item 1, which was affirmed as final order on January 13, 2016, with respect to the workplace located at 3644 Abbeyville, Medina OH 44256.
