29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  At the front side of the residential building, on or about July 31, 2018, an employee was exposed to fall hazard of 10 feet, 7 inches while installing windows from a 3.5:12 pitch roof without using a fall protection system.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:   (a) At the left side wall of the building at 18012 Woodland View Dr., Lutz, FL, on or about July 31, 2018, an employee installing tapcon screws did not use the 22 foot Werner Ladder at the proper angle, in that the distance between the foot of the ladder to the top support on the wall was eight feet, three inches from the wall, exposing them to a 14 feet fall hazard.  (b)  At the right side wall of the building at 18012 Woodland View Dr., Lutz, FL, on or about July 31, 2018, an employee installing tapcon screws did not use the 22 foot Louisville Ladder at the proper angle, in that the distance between the foot of the ladder to the top support on the wall was nine feet, seven inches from the wall, exposing them to a 14 feet fall hazard.
