29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: The employer is failing to protect employees from fall hazards while working on commercial roof structures. This was most recently observed at 7575 Crown Pointe Ave, Omaha, Nebraska on July 30, 2018 when employees were observed replacing the roof on a commercial storage structure that had an eave height of 9'2" and a pitch of 4:12 and there were no protective systems in use to protect from fall hazards.

This was most recently observed at 7575 Crown Point Ave, Omaha, Nebraska on July 30, 2018 when employees were observed using a ladder to access the roof of a commercial structure and the ladder did not extend at least 3' past the roof edge.
