29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about July 26, 2018, Aspen Contracting, Inc., as controlling employer, did not ensure that employees of Luis Torres dba Torres Roofing, subcontractor, were protected from fall hazards. Frequent and regular inspections of the job site were not conducted by a competent person to ensure that safety hazards were identified and mitigated. Workers were not using their personal fall arrest system and ladder properly and were exposed to fall hazards of approximately 8 feet.
