The employer is failing to ensure that employees eyes and face are protected from flying particles while working on a residential roof. This most recently occurred on July 31, 2018 on the jobsite located at 2412 Alexandra Rd, Papillion, NE 68133. Two employees were observed using hand-held pneumatic nail guns to install roofing material without any form of eye or face protection.

This most recently occurred on July 31, 2018 on the jobsite located at 2412 Alexandra Rd, Papillion, NE 68133. The company owner and four employees were re-roofing a single-story residential house and not utilizing any form of fall protection. The house has a five in twelve steep sloped pitch and ground to eve height is approximately nine and a half feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This most recently occurred on July 31, 2018 on the jobsite located at 2412 Alexandra Rd, Papillion, NE 68133. The owner and four employees are using a fiberglass extension ladder to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about nine and a half feet above the ground.
