29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about July 26, 2018, located at the Disk Builder platform, employees were inspecting working conditions while on the platform without any means of fall protection, thereby exposing the employees to a fall hazard of approximately six feet seven inches.    b. On or about August 8, 2018, located on the mezzanine in Building C, an employee was installing an expansion joint on an exhaust blower from an elevated surface without any means of fall protection, thereby exposing the employee to a fall hazard of approximately seven feet four inches.

29 CFR 1926.502(d)(6): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged;    29 CFR 1926.502(d)(6)(i): directly to webbing, rope or wire rope:    a. On or about August 8, 2018, located on the 1st floor in Building C, an employee was welding a section of pipe within ceiling I-beams with a Miller Turbo Lite retractable that had its snaphook tied back onto its webbed strap. The employee was exposed to a maximum fall hazard of approximately 15 feet if the snaphook would become disengaged from the strap during a fall arrest condition.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a. On or about July 26, 2018, located in on the mezzanine in Building C, employees used an aluminum "Super Scala Prima" ladder to ascend and descend onto the Disk Builder platform while the ladder did not extend at least three feet above the landing surface, exposing the employees to a maximum fall hazard of approximately six feet seven inches.
