29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) Orefield Cold Storage & Distribution Center, Inc. - On or about August 1, 2018, the employer did not document that the door entrance from the maintenance room into the compressor / engine room complies with their chosen recognized and generally accepted good engineering practices of IIAR 2, in that the door is not tight fitting and does not meet requirements of IIAR 2- 1999 section 6.3.1.5. Orefield Cold Storage & Distribution Center, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.119(d)(3)(ii), which was contained in OSHA inspection number 1188179, citation number 01, item number 01a, and was affirmed as a final order on July 13, 2017, with respect to a workplace located at 3824 Route 309, Orefield, PA 18069.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from materials stored on damaged racks. a) Orefield Cold Storage & Distribution Center, Inc. - On or about August 1, 2018, storage of material created a hazard in that steel storage rack location # 03H026 A & # 03H027 A had a cross bracing that was cut and removed and the rack was not immediately unloaded and the damaged portions were not repaired or replaced. b) Orefield Cold Storage & Distribution Center, Inc. - On or about August 1, 2018, storage of material created a hazard in that steel storage rack location # 03H026 A & # 03H027 A had a cracked vertical column at a weld and the rack was not immediately unloaded and the damaged portions were not repaired or replaced. c) Orefield Cold Storage & Distribution Center, Inc. - On or about August 1, 2018, storage of material created a hazard in that steel storage rack location # 03H026 B & # 03H027 B had two seriously bent cross bracings and the rack was not immediately unloaded and the damaged portions were not repaired or replaced. d) Orefield Cold Storage & Distribution Center, Inc. - On or about August 22, 2018, storage of material created a hazard in that steel storage rack location # 03H036 A & # 03H037 A had two seriously bent, and twisted, vertical members and the rack was not immediately unloaded and the damaged portions were not repaired or replaced. e) Orefield Cold Storage & Distribution Center, Inc. - On or about August 22, 2018, storage of material created a hazard in that steel storage rack location # 03H036 A & # 03H037 A had two seriously bent, and twisted, cross bracing and the rack was not immediately unloaded and the damaged portions were not repaired or replaced.
