29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: JOBSITE: The employer did not ensure that employees accessing a balcony surrounded by a perimeter guardrail approximately 28 inches to 3 feet high were provided with a ladder or other safe means of access. On or about July 31, 2018, one of employees fell to the ground while attempting to climb over the perimeter guardrail of a balcony. The employee sustained a broken right arm and strain to the left leg.

JOBSITE: The horizontal distances from the top supports to the bases of the three (3) extension ladders were greater than one-quarter (1/4) of the working lengths of the ladders.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: JOBSITE: The employees utilizing aluminum extension ladders to access upper landing surfaces were not provided with information and training in the proper procedures to minimize the hazards associated with ladders. Shoreline Painting & Drywall, Inc. was previously cited for a violation of this Occupational Safety and Health standard, 29 CFR 1926.1060(a), which was contained in OSHA Inspection Number 1094307, Citation Number 01, Item Number 1b, and was affirmed as a final order on December 11, 2015, with respect to a workplace located at 2770 Summer Street, Stamford, CT 06905.
