29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about August 1, 2018, during the installation of a waterline at a worksite located at the Harvard Avenue Ohio and Erie Canal Towpath Trailhead, Cleveland, Ohio, the employer did not ensure three employees working inside a trench with a depth varying from approximately 6 to 6.5 feet deep, were protected from cave-ins by an adequately designed protective system. The soil would have been classified as Type B .
