29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    125 Gloucester Avenue, Gloucester, MA, Building 1, VTL section    VTL and CNC machines were not provided guarding to protect the operator and other employees in the machine area from hazards created by rotating parts of the machines. This includes, but is not limited to the following instances:    a) On or about August 1, 2018, employees were exposed to hazards on the Gray VTL Shop ID# 10038. The rotating table was not guarded by a method of guarding that prevented employees from contacting the table.     b) On or about August 25, 2018, employees were exposed to hazards on the Summit VTL Shop ID# 13104. The rotating table was not guarded by a method of guarding that prevented employees from contacting the table.     125 Gloucester Avenue, Gloucester, MA, Building 2, main shop floor    c) On or about August 27, 2018, employees were exposed to hazards on the Okuma OSP700M CNC Shop ID# 13233. The CNC door interlock was found to be defeated, in that the key was removed from the sliding door and placed in the interlock, allowing the machine to operate with the door in the open position, exposing employees to struck by hazards.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(b)(6)(i): Two-hand trip(s) on mechanical power press(es) using full revolution clutches did not have the individual operator's hand controls arranged by design and construction to require concurrent operation of the individual operator's two hand controls:    125 Gloucester Ave, Gloucester, MA; Basement Glass Shop    Employees operating mechanical power presses were exposed to amputation hazards due to two hand trips that were not arranged by design and construction to require both employee hands, concurrently, to operate the machine. This includes, but is not limited to the following instances;    a) On or about August 1, 2018, the Moly Press Shop ID 24101 was operated without having two hand trips that required concurrent operation of the operator's two hand controls. The machine was able to be cycled with one control.     b) On or about September 6, 2018, the Moly Press Shop ID 24103 was operated without having two hand trips that required concurrent operation of the operator's two hand controls. The machine was able to be cycled with one control.     c) On or about September 5, 2018, the V & O press, shop ID 14103 was operated without having two hand trips that required concurrent operation of the operator's two hand controls. The machine continued to run without releasing both trips in-between cycles.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

This includes, but is not limited to the following instances:    a) On or about July 28, 2018, the Bliss 14108 press was operated without a control system that would prevent the press from being operated by foot pedal in a fashion that would expose the operator to an amputation hazard in the point of operation. The operator suffered a fingertip amputation.     b) On or about September 5, 2018, the Federal power press shop ID 14102 press was operated without a control system that would prevent the press from being operated by a two hand control in a fashion that would expose the operator to an amputation hazard in the point of operation.
