29 CFR 1926.416(e)(1): At this job site, the orange 50 ft extension cord, being used to energize the Dewalt angle grinder, had damaged insulation at the female plug and at 2 other areas of the cord. A) 19 E Morris Ave Linden, NJ 07036 The 50ft orange extension cord's insulation was pulled away from the female end of the plug and duct tape was being used to cover other defects in the cord. Occurred on or about 09/05/18
