29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) On or about July 18, 2018, the employer failed to initiate or maintain frequent and regular inspections of the job site to identify hazards such as; workers working over six feet above a lower level without fall protection, a hand held electrical circular saw with a damaged electrical cord and workers using a ladder to access a second floor porch where the ladder did not extend three feet beyond the level to be accessed.
