29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employees working from an aerial lift: (a) New System Engineered Metal Building under Construction - Two employees used a JLG aerial lift to access the roof, which had a peak (maximum) height of 28.5 feet above the ground. The employees did not wear harnesses and lanyards and attach to the boom or basket of the lift, exposing them to the hazard of falling to a lower level, on or about July 30, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Building under Construction at 213 Poplar Street, Hanover, Pennsylvania - An employee suffered injuries requiring hospitalization after falling 28.5 feet from the roof of the building, on July 30, 2018. The employer failed to report this work-related in-patient hospitalization to OSHA within 24 hours. (The employer reported the in-patient hospitalization to OSHA on August 1, 2018.) Corrected during the inspection.

29 CFR 1926.760(c)(3): The boundaries of a Controlled Decking Zone (CDZ) were not designated and clearly marked: (a) New System Engineered Metal Building under Construction - Two employees worked installing the metal roofing, making initial connections, on a system engineered metal building, which had a peak (maximum) height of 28.5 feet above the ground. The employees did not have a controlled decking zone designated and clearly marked, on or about July 30, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
