29 CFR 1910.179(j)(2)(iv): Hoist chains, including end connections, for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations. Visual inspection daily; employer did not conduct a monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected: Throughout the facility. On or about August 1, 2018, and at times prior thereto, employees were exposed to a struck-by hazard, while operating the Beta Systems C-Weld steel wire mesh weaving machine, without ensuring a documented monthly inspection of the crane hoist chains was accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 1, 2018, and at times prior thereto, employees were exposed to a caught-in hazard as they passed the area, from the C-Weld wire positioner machine being unguarded.
