29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative.      On or about July 24, 2018 and at times prior there to, the employer did not ensure that a energy control program consisting of energy control procedure(s), employee training, and periodic inspections for employees performing maintenance work on machinery and equipment.  This exposed employees to struck-by, caught in, and amputation hazards.

On or about July 24, 2018 and at times prior there to, the employer did not ensure lockout or tagout devices were affixed to each energy isolating device by authorized employees before an employee began to exchange blades on a planer.  This exposed employees to amputation hazards.

On or about July 24, 2018 and at times prior thereto, the employer did not ensure that all moving parts on the wood planers feeder conveyor were guarded.  This exposed employees to caught in and amputation hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.       An amputation of an employees hand, occurring on or about July 24, 2018, was not reported to the local OSHA area office within the 24-hour reporting requirement.
