29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about August 2, 2018, in the production area, employees were exposed to slip, trip and fall hazards from slippery surfaces created by surfactant products manufactured in the work area. The employer failed to provide and require the use of slip resistant footwear when required to access the production floor.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a. On or about August 2, 2018, in the facility, the employer had conducted an inadequate personal protective equipment assessment. The personal protective equipment assessment did not define what type of special personal protective equipment was necessary and during what specific tasks in the maintenance and other departments. The employer required more protection during specific tasks than which was included in the assessment.
