29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:  a) 801 Victory Blvd, Portsmouth, VA 23702: On or about August 27, 2018, and times prior thereto, an employee that performed abrasive blasting was exposed to noise levels in excess of the allowable 8-hour time weighted average (TWA) of 85dBA. This was observed on August 27, 2018, where noise sampling performed up to 224 minutes indicated that the employee was exposed up to an approximately 1631.2 percent noise dose which is equivalent to 110.1 dBA. Zero exposure was assumed for the 256 minutes not sampled. The employer did not provide an at-least annual audiogram, exposing the employee to potential hearing loss.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1915.77(c): When employees were working aloft, or elsewhere at elevations more than 5 feet above a solid surface, the employer did not provide either scaffolds or a sloping ladder, meeting the requirements of Subpart E-Scaffolds, Ladders, and Other Working Surfaces, to afford safe footing, and the employees were not protected by safety belts and lifelines meeting the requirements of 1915.159 and 1915.160:  a) 801 Victory Blvd, Portsmouth, VA 23702, Big Boat Shop: On or about August 27, 2018, and times prior thereto, the employer did not provide scaffolds, sloping ladder, or safety belt and lifeline for employees that worked at the edge of boat decks up to approximately 7 feet above the concrete floor, exposing employees to fall hazards.
